Hospice services of lake county is seeking volunteers for our school-based grief counseling program. Join Wings of Hope today to help students who are experiencing grief, or showing the following signs of grief: Deep sadness, excessive worry, feeling worthless, or difficulty concentrating. Our professional team is committed to providing exceptional Comfort Care to patients, family members and caregivers when you are no longer seeking a cure, but hoping for comfort and compassion.

Lake Palliative Care

Palliative Care Services provide an added layer of support to help you manage the challenges that accompany serious illness.

Your quality of life is more than your medical problems and treatments, and yet, when faced with a serious illness, life can become more stressful.

Our professional, compassionate Palliative Care team is here to help so you can focus on what’s most important to you.

Enhanced Care Management (ECM)

Enhanced Care Management provides a buffer of support for our community who have Partnership Health Insurance but may not qualify for Palliative Care or Hospice Care.

We understand that hard times affect us all, and the lack of a qualifying diagnosis shouldn’t impact the resources available to you.

Grief and Healing

Grief Support is available to any community member, regardless of receiving Hospice or Palliative Care. You are encouraged to explore which Grief Support service may best meet your needs.

Grief is a very personal journey, traveled in different ways and at different times and places. Our professional grief counselors and trained volunteers ensure that you don’t have to make the journey alone.
